No Therapist-Client Relationship Through This Website
Using this website, reading its content, or sending us a message through the contact form does not create a therapist-client relationship. A therapist-client relationship is only formed when you and your therapist have both agreed to begin treatment, completed the required intake paperwork, and signed a consent form.
Any information you send to us through the website contact form, email, or other online communication before becoming a client is not protected by therapist-client confidentiality. Please do not share sensitive personal or health details through these channels until a formal therapy relationship has been established.
Appointments and Cancellation Policy
When you schedule an appointment with Creative Pathways Therapy, LLC, you are reserving a specific time for your care. We ask that you arrive on time for all appointments.
If you need to cancel or reschedule, please give us at least 24 hours notice. This allows us to offer that time slot to another person who needs care.
If you cancel with less than 24 hours notice or do not show up for your appointment, you may be charged a cancellation fee. This fee is your responsibility and is typically not covered by insurance.
We understand that emergencies happen. If something unexpected comes up, please contact us as soon as you can, and we will do our best to work with you.
Telehealth Consent
Creative Pathways Therapy, LLC offers telehealth (video) sessions as an option for clients. By choosing to use telehealth services, you understand and agree to the following:
- Telehealth sessions use secure, encrypted video technology that meets HIPAA standards.
- While we use secure technology, no electronic communication is completely risk-free. There is a small chance that information could be seen by others due to technical problems.
- You are responsible for finding a private, quiet location for your telehealth session.
- A stable internet connection is needed for the best experience. If technical problems occur, your therapist may call you by phone to continue or reschedule.
- Telehealth may not be appropriate for all situations. Your therapist will let you know if in-person sessions would be a better fit for your needs.
- You have the right to stop using telehealth at any time and switch to in-person sessions.
A separate telehealth consent form will be provided before your first video session.
Payment and Insurance
Creative Pathways Therapy, LLC accepts payment for services through various methods. Payment details will be discussed before your first appointment.
If you use insurance, we will work with your insurance company to verify your benefits before treatment begins. However, you are ultimately responsible for understanding your insurance coverage, including copays, deductibles, and any services that may not be covered.
If your insurance does not cover a service, or if you choose not to use insurance, you will be responsible for the full cost of that service. Payment is due at the time of your appointment unless other arrangements have been made in advance.
If you have questions about fees or payment options, please contact us before your appointment. We want to make sure there are no surprises when it comes to cost.
